The Dialectics of Disaster: Transforming Macondo’s Lessons into Live Operations

This article demonstrates how Eye Opener can be used to convert formal safety recommendations into operational decisions.

The 2010 Macondo blowout report (available here) is one of the most detailed safety analyses ever written—yet it still leaves a gap between recommendations and real operations. In this article, we expose that gap by explicitly modeling how rules are violated (via using antitheses), and by deriving concrete steps that are not stated in the report but are critical for safer operations.
